Hilton East Assisted Living currently has multiple Full-time/part-time openings for a Home Health Aides/PCA level 2 at our assisted living facility located in Hilton, NY.

SHIFT DETAILS:

  • Full-Time/Part-time:
  • Saturday and Sunday shifts with differential pay



Responsibilities and Functions

  • Works under the supervision of the Registered Nurse and/or Licensed Practical Nurse in accordance with the New York State Department of Health regulations.
  • Provides clients and/or residents with assistance with ADLโ€™s and housekeeping tasks, as dictated by the Nurse and/or Patient Care Plan
  • Able to follow the Nursing Care Plan to complete the daily activities of the client and/or resident, as well as completion of the aide activity sheet.
  • Complete all documentation in a neat and timely matter
  • Observe the client and/or resident, and report changes appropriately to RN or LPN
  • Attend required In-service training and HHA meetings.
  • Must complete 12 In services per year to stay certified.
  • Performs tasks and duties assigned or requested by shift supervisor.
  • Exhibits necessary interpersonal skills to work well with clients and families.
  • Work hours as scheduled by supervisor.

Licensure/Certification:

  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification in New York State.
  • CPR and First Aid certification (or willingness to obtain).

Experience:

  • Prior experience in an assisted living, nursing home, or home care setting preferred.
  • Experience in administering medications or assisting with medication management preferred.

Skills:

  • Ability to work independently and as part of a team.
  • Strong organizational skills and attention to detail, especially in documentation and medication administration.
  • Excellent communication skills with residents, families, and the care team.

Preferred:

  • Experience in an assisted living or healthcare setting.
  • Familiarity with electronic health records (EHR) or medication management systems.

Physical Requirements:

  • Ability to lift, move, and assist residents in a safe and respectful manner.
  • Ability to stand, walk, bend, kneel, and perform physical tasks throughout the workday.
  • Must be able to handle insulin checks/injections.

Work Conditions:

  • Full-time and part-time positions available, including day, evening, overnight, and weekend shifts.
  • Occasional lifting and physical exertion may be required.
