Position Summary

The Medical Social Worker is responsible for providing psychosocial support to hospice patients and their families. This role involves assessing patient and caregiver needs, coordinating resources, offering counseling, and ensuring services align with the individualized plan of care.


Key Responsibilities
  • Conduct timely psychosocial assessments of patients and families/caregivers
  • Develop and support individualized care plans based on patient needs
  • Provide counseling to patients, families, and staff related to stress and coping challenges
  • Maintain accurate and timely documentation in accordance with care plans
  • Assist with applications for Supplemental Security Income (SSI), Medicaid, Hospice benefits, and waiver programs
  • Coordinate community resources including social, health, pharmaceutical, and welfare services
  • Serve as a liaison between patients, families, caregivers, and community agencies
  • Deliver exceptional service to patients, referral sources, and interdisciplinary team members

Qualifications

Education:

  • Bachelors degree in Social Work required (Masters degree preferred)
  • Acceptable alternative degrees: Psychology, Sociology, or related field (must meet supervision and experience requirements)

Licensure:

  • Current Ohio Social Work Licensure required (LSW, LISW, or LPSW-S)

Experience:

  • Minimum of 1 year of healthcare or hospice experience
  • Experience supporting patients and families in a clinical or community-based setting preferred

Other Requirements:

  • Valid drivers license and reliable, insured transportation
  • Strong interpersonal, communication, and organizational skills
