Registered Nurse (RN) โ€“ Admissions Nurse | 7 On / 7 Off

Glen Haven Health & Rehabilitation is seeking an experienced and compassionate Registered Nurse (RN) Admissions Nurse to join our clinical team on a 7 On / 7 Off schedule.

As our RN Admissions Nurse, you'll play an important role in welcoming new residents and helping ensure a smooth transition into our skilled nursing and rehabilitation community. You'll coordinate the clinical admission process, complete nursing assessments, review physician orders, communicate with families, and collaborate with our interdisciplinary team to establish each resident's plan of care.

The 7 On / 7 Off schedule provides a unique opportunity for an RN looking for a full-time nursing career with a consistent schedule and extended time off between rotations.

RN Admissions Nurse Responsibilities
  • Coordinate and complete the nursing admission process for new residents.
  • Perform comprehensive admission assessments and document resident conditions.
  • Review hospital records, discharge information, medication lists, and physician orders.
  • Ensure physician orders are accurately entered, communicated, and implemented.
  • Complete required admission documentation accurately and within established timeframes.
  • Identify immediate clinical needs and communicate concerns to nursing leadership and physicians.
  • Collaborate with physicians, pharmacy, therapy, dietary, social services, and other interdisciplinary team members.
  • Assist in developing individualized resident care plans.
  • Communicate with residents and families throughout the admission and transition process.
  • Coordinate with nursing units to ensure staff have the information needed to provide appropriate resident care.
  • Maintain accurate electronic medical records (EMR) and ensure compliance with facility, state, and federal requirements.
  • Help create a welcoming, organized, and positive admission experience for residents and their families.
Qualifications
  • Current Alabama Registered Nurse (RN) license in good standing.
  • Current CPR/BLS certification.
  • Strong nursing assessment and clinical documentation skills.
  • Excellent communication, organization, and time-management abilities.
  • Experience in skilled nursing, rehabilitation, long-term care, post-acute care, admissions, or transitional care preferred.
  • Experience with electronic medical records (EMR) preferred.
  • Ability to coordinate multiple aspects of resident care while communicating effectively with residents, families, physicians, and healthcare professionals.
