The Position:

Therapy Care Pharmacists utilize national and local resources to facilitate high quality care of a patients with specialty pharmacy needs. They collect and assess clinical data from patients, implement and monitor care plans, communicate with prescribers to obtain orders, report outcomes, make therapeutic recommendations, and dispense prescription medications and supplies, and supervise support activities of therapy care specialists in accordance with all laws, regulations, and policies and procedures set forth by state and federal law, accrediting bodies, and Soleo Health. Responsibilities include:

  • Collect clinical data for patients, caregivers, and nurses and assess data to monitor efficacy and safety of a prescribed specialty therapies
  • Collaborate with prescribers to implement or modify existing treatment plans
  • Act as a subject matter expert and clinical knowledge resource to patients, prescribers, referral sources, and fellow team members
  • Coordinate activities with local or national pharmacy team members, including pharmacists, pharmacy technicians, nursing, intake/admissions, reimbursement, and sales staff to coordinate to ensure an efficient customer focused approach and optimal clinical patient outcomes
  • Documentation of all patient interaction and care coordination in the designated system of record for clear and consistent communication
  • Dispense specialty medication and supplies
  • Promotes teamwork, proactivity, leadership, service, mentorship, positivity, and optimism to strengthen staff and contribute to excellent work environment and customer service
  • Embraces Soleo’s Quality Assurance and Performance Improvement initiatives, and plays an active role in supporting these programs
  • Assist in orientation and training of new pharmacy staff
  • Supervises activities of therapy care specialists and other support personnel
  • Ensures all pharmacy activities and documentation comply with state and federal law, accreditation body standards, Soleo Health policies and procedures, and payor, manufacturer, and referral source requirements
  • Obtains additional state pharmacy licenses as requested
  • Performs other related duties as assigned and required

Schedule:

  • Full-time, 40 hours per week, Monday to Friday, 9:30am - 6:00pm
  • Must be willing to participate in an on-call rotation
Requirements:
  • Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience
  • Post graduate residency and/or training
  • Prior experience in home infusion and/or specialty pharmacy preferred with a focus on ultra-rare/orphan diseases
  • Applicable active state licensure
  • Additional specialty certification or training may be required
  • Prior experience with CPR+ pharmacy computer system preferred
  • Excellent clinical and customer service skills
  • Basic to intermediate efficiency with Microsoft Excel, Word and Outlook.
