What You'll Do:
  • Provide on-call nursing coverage as scheduled (5:00 PM - 8:30 AM, 7 on / 7 off)
  • Respond to patient/caregiver calls and visits in a timely manner, including medical or emotional crises and patient deaths
  • Conduct comprehensive assessments
  • Communicate effectively with attending physicians, GSH staff, and other agencies to coordinate optimal care
  • Educate patients and families on hospice philosophy, disease progression, symptom management, and care plans
  • Document all patient interactions, visits, and assessments in accordance with GSH policies and electronic medical records
  • Report patient status changes to the Interdisciplinary Team to ensure continuity of care
  • Assist RN Case Managers with after-hours visits and patient care needs
  • Participate in required meetings, staff in-services, and continuing education to maintain clinical competencies

What We're Looking For:
  • Clinical Expertise - Graduate of an accredited nursing program with current Virginia RN licensure
  • Experience - Minimum 2 years in nursing, including hospice, home health, med/surg, acute care, or geriatric nursing
  • Critical Thinking & Decision-Making - Strong judgment, problem-solving, and ability to handle complex patient situations
  • Communication Skills - Excellent verbal, written, and interpersonal skills; able to work collaboratively with the IDT
  • Technology Skills - Experience with EMR documentation and general computer applications
  • Compassion & Professionalism - Provides patient-centered care with empathy, discretion, and integrity

Qualifications:
  • Graduate from an accredited educational institution
  • Current Virginia RN license
  • Minimum of 2 years of relevant nursing experience
  • Experience with hospice, home health, or inpatient care
  • Valid Virginia driver's license and reliable transportation
  • Ability to work independently and respond appropriately to after-hours patient needs
  • Commitment to professional growth and continuing education
