All Care Home Health is a Medicare-certified home health agency established in 1998. Our mission is to provide compassionate and effective health care in the most comfortable, secure environment for the patient: home.
Located in Danville, Virginia, we serve patients of all ages who reside in Danville and Pittsylvania County, and in parts of Halifax and Henry Counties. Our services include skilled nursing, physical therapy, occupational therapy, speech-language pathology, and home health aide.
Job Description
The Registered Nurse (RN) is responsible for providing skilled nursing, intervention, and supportive care to patients in the home setting in accordance with the All Care Home Health Plan of Care for each patient.
Each Staff Nurse (RN) will be assigned shifts during the agency's business hours (Monday through Friday, 8am to 5pm). Each Staff Nurse (RN) will also be assigned on-call shifts. On-call shifts are defined as follows: An after-hours shift is Monday through Friday, and typically begins at 5pm and extends until 8am the following day. A weekend shift is Saturday or Sunday, and typically begins at 8am and extends until 8am the following day. A holiday shift may be a weekday, Saturday or Sunday, and typically begins at 8am and extends until 8am the following day.
Specific Responsibilities
  • Assume primary responsibility for All Care Home Health total census during assigned shift.
  • During an on-call shift, answer all phone calls received on behalf of the agency, and follow up according to agency policies and procedures.
  • Provide skilled nursing, intervention, and supportive care to patients in the home setting.
  • Educate primary caregivers, volunteers, and employed caregivers to provide care for patients as indicated.
  • Maintain regular communication with agency staff and the patient's referring physician and/or case manager concerning patient/family care needs and response to interventions.
  • Maintain up-to-date patient records so that problems, goals and interventions are accurately and clearly stated and changes are reflected as they occur.
  • Complete documentation within agency guidelines and to meet regulatory requirements. Submit completed documentation within one business day after on-call services are provided.
  • Accept referrals for new patients in accordance with the agency's policies and procedures.
  • Communicate regularly with agency staff to review problems or unique issues from on-call hours
  • Participate in the agency's orientation and in-service training programs for professional staff
  • Participate in agency's performance improvement activities and marketing and community programs as requested, to promote professional growth and understanding of home health care
  • Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies
  • Perform specific assignments as directed

Qualifications
Qualifications
  • Possess a current license to practice as a Registered Nurse (RN) in the Commonwealth of Virginia or the State of North Carolina. Preferably possess a Bachelor of Science degree in nursing.
  • Minimum two years of varied work experience as a registered nurse with preferably one of the two years in either an acute, oncology, home health and/or hospice setting.
  • Possess a valid driver's license, valid auto insurance, and an automobile for field use
  • Ability to remain within the agency service area or within 30 miles of the agency office while on-call
  • Ability to provide skilled nursing services and complete documentation within the agency's productivity standards
  • Demonstrated ability to assess and respond to the needs of patients and families in varied settings
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles

Physical Requirements
  • Ability to turn, reposition, and assist patients to transfer and ambulate
  • Must be able to bend, stoop, push and pull
  • Must be able to lift a minimum of 25 pounds
  • Requires manual dexterity sufficient to provide hands-on care to the patient under variable conditions and settings
  • Must be able to operate standard office machines such as fax, telephone, computer, etc.
  • Requires normal ranges of hearing and vision
  • Must be free of communicable diseases
  • Must be able to communicate clearly in English verbally and in writing
