The Renaissance of Richfield/ Bath located in Richfield, Ohio is currently seeking both full-time Memory Care Director LPN/RN!

Responsibilities:

  • Assists Executive Director with move-in/move-out decisions and in the development of individualized service plans of Members joining the community. Ensures continuity of care for Members, and the provision of the highest quality of ongoing care.
  • Supervises orientation and ongoing training of Nursing Department Staff (Care Partners and Charge Nurses). Initiates and maintains nurturing work environment that fosters professional growth and the highest quality care for Members. Responsible for performance evaluation of Department with input from Assistant Director and Charge Nurses.
  • Supervises nursing assessments of Members/Weekly Member Review with Staff and Therapists.
  • Create and maintain positive and professional relations with families of Members, addressing concerns of family and power of attorney; advises on change of care plans and levels of care.
  • Coordinates with Ancillary Service providers to develop clinical protocols to manage acute medical and nursing needs
  • Serves as a role model for Charge Nurses and Care Partners and other staff by demonstrating approaches and techniques that enable Members to be independent for a long as possible.
  • Oversees the delivery of services specified in members’ individual service plans.
  • Provides on-call nursing support for acute medical problems, medication orders, and other emergencies that may occur.
  • Manages infection control; coordinates safety and sanitation control with Maintenance Manager.
  • Ensures accurate documentation of health and personal care services provided in member record.
  • Develops a working knowledge of the applicable state regulations and ensures member services are in compliance with those regulations.
  • Hire, develop and retain a compassionate, efficient and professional staff that focuses on meeting member needs.

Requirements:

  • Must have a valid and current LPN/RN state license
  • 1-2 years of nursing experience
  • Must have valid CPR certificate
  • Must have effective communication skills
  • Demonstrate supervisory skills
  • Must have computer skills
