As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, versatile team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

Your Role & Responsibilities
  • You will provide comprehensive care to seniors following clinical standards.
  • You will collaborate daily with your care team to ensure coordinated, high-quality care.
  • You will manage referrals, hospital/SNF coordination, DME, and home health services.
  • You will document care with support from quality-based coders.
  • You will participate in on-call rotation and support growth plans at your center.
  • You will have experience providing value-based care.
What We're Looking For

Required:

  • You will have a Master's Degree in Nursing or completion of a PA program with board certification.
  • You will have an active, unrestricted NP or PA license in the state of practice.
  • If selected for this role, we will require you to screen for TB.

Additional Information:

  • You'll report to the Area Medical Director.
  • This is an in-person role.
  • You will support 3 of our outpatient clinics in the area.

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
