We're currently seeking a passionate and skilled Registered Nurse (RN) to join our ever-expanding Hospice team in the Pottsville, PAand surrounding areas. This role offers the chance to directly provide patient-centered care in the home that focuses on comfort, dignity, and support during life's most challenging moments, truly making a difference in the lives of our patients.

What You'll Do:
  • Conducting Start of Care comprehensive assessments
  • Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
  • Preparing accurate, orderly, and timely documentation
  • Coordinating care with patient and provide proper reports to Case Manager
  • Provides health care instructions to the patient/family as appropriate per assessment and plan

Qualifications
About You:
  • An active RN license in the state in which practicing (Minimum of 1 year of experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Compassionate and empathetic approach to patient care
  • Ability to work independently and as part of a team
  • Knowledge of hospice regulations and standards of care
  • Reliable transportation and willingness to travel
