Job description

Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist in Silverdale, WA providing home health services where you will help patients recover from injuries and manage chronic conditions while working across Kitsap County.
Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
  • Active Washington State PT license required to start the assignment.
  • BLS certification required from AHA or ARC.
  • Experience with home health settings preferred.
  • Experience using Wellsky and OASIS documentation preferred.
  • Familiarity with OASIS SOC, ROC, Recert, and Discharge processes preferred.

Preferred Qualifications
  • Prior experience covering territory-based caseloads.
  • Comfort with productivity expectations measured in OASIS and visit points.
  • Experience collaborating with nurses who complete portions of OASIS SOC documentation preferred.

Assignment Details
  • Schedule: Flexible. Options include 5x8s or 4x10s.
  • Territory: Kitsap County, with primary coverage in North Kitsap (Poulsbo, Kingston, Bainbridge) and occasional county-wide visits.
  • Mileage: Standard mileage paid; higher rate provided for travel over 45 miles per day.
  • Weekly productivity expectation: 30 points per week (approximately 6 points per day; 75% productivity expectation).
  • EMR: Wellsky and OASIS.
  • Weekend rotation: no.
  • On-call requirements: no.

As a travel Physical Therapist in home health you will provide skilled evaluations, supervisory visits, and ongoing therapy visits while documenting care using OASIS and Wellsky. Licensed nurses support SOC documentation to reduce time spent on initial assessments. Productivity is tracked via OASIS and visit point values for evaluations, visits, recerts, and discharges.
