PRN Home Health RN (Registered Nurse)

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in east Pasco county (including Wesley Chapel, Odessa, Zephyrhills, Land O' Lakes, Dade City, etc.).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in east Pasco county (including Dade City, Land O' Lakes, Wesley Chapel, Odessa, Zephyrhills, and surrounding areas)

Schedule: 10+ home health visits per week. Required weekend/holiday on-call rotations.

  • Develop, initiate, and revise individualized plans of care as patient needs evolve
  • Deliver skilled nursing services and treatments as outlined in the plan of care

  • Collaborate with the interdisciplinary team to ensure seamless, holistic care
