Job Summary

The On-Call Hospice Registered Nurse (RN) is responsible for providing after-hours nursing support to hospice patients and families. This role focuses on responding to urgent patient needs, providing symptom management, and offering emotional support during evenings, nights, weekends, and holidays.

The On-Call Hospice RN works closely with the hospice care team to ensure continuity of care and timely response to patient and family needs.

Qualifications

Required

  • Active Registered Nurse (RN) license in the State of Connecticut
  • Current BLS certification
  • Minimum of 2 years hospice nursing experience

Preferred

  • Home health experience
  • Strong clinical assessment and triage skills
What You’ll Do
  • Respond to after-hours calls from hospice patients and families
  • Provide clinical triage and nursing guidance over the phone when appropriate
  • Conduct in-person visits for urgent patient needs when required
  • Assess patient symptoms and provide interventions to manage pain or discomfort
  • Provide emotional support and guidance to patients and caregivers
  • Communicate patient updates to the hospice care team and document all interactions
  • Coordinate care with physicians and other members of the hospice team
  • Ensure continuity of care and timely follow-up for patient needs

Salary: $500 per week + fee per visit
