Registered Nurse (RN) On Call

Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996. Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas. Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas.

$5,000 Sign-on bonus

Position Summary

The On-Call RN provides emergency support and skilled visits to patients on an on-call basis during non-standard work hours.

Essential Functions
  • Make emergency on-call visits demonstrating the skills, knowledge, and judgment necessary to implement the nursing care plan; document appropriately to meet Medicare conditions of participation; provide quality care through symptom management and emotional support.
  • Participates in weekly team conferences by contributing necessary information and documentation.
  • Responsible for Hospice patients during non-office hours, providing a feeling of security through phone communication and skilled visits as indicated.
  • Assumes responsibility for attendance, dependability, and personal appearance with minimal supervision.
  • Must be capable of handling interpersonal relationships, able to communicate effectively, and demonstrate professional development.
  • Must have the ability to use, store, and care for equipment, use safety in work habits, and respond properly in crises.
  • May be required to evaluate appropriateness for hospice care and admit patients.
Qualifications
  • Registered Nurse in the state of Texas
  • Hold a valid driver's license and have a dependable means of transportation
  • Minimum 2 years of RN experience required
  • 6+ months in an On-Call role required
  • Crisis management experience strongly preferred
  • 1 year Hospice or Home Health experience strongly preferred
  • Proficient computer skills are required; Microsoft Office suite experience preferred.
