As a Case Manager, you'll provide full-scope case management services to patients and their families - including assessment, discharge and transitional planning, care coordination, utilization review, and community outreach. You'll partner with the multidisciplinary care team to identify discharge barriers early, build effective plans of care, and ensure smooth transitions to external providers and services.
This is a role with both clinical depth and operational impact. You'll work cross-functionally to manage financial risk to families, navigate payer authorizations, and contribute to performance improvement initiatives that strengthen patient outcomes.
Earnings$85,000-$96,000 based on experience salary
Location - In Office Schedule - Schedule: Monday-Friday, day shift, salary/exempt
- On-call: Weekend rotation 3-5 times per year, plus a minor/major holiday rotation
Job TypeDuties - Initial patient and family assessments to identify discharge barriers and care needs
- Development and execution of discharge and transitional plans of care in partnership with the multidisciplinary team
- Clinical reviews and utilization management using nationally recognized criteria
- Payer coordination - authorizations, denials, and communication around level-of-care changes to limit financial risk to families
- Documentation in accordance with departmental policy and regulatory standards
- Participation in shared leadership, performance improvement initiatives, and community engagement
Requirements - 5+ years of nursing experience with at least 3 years in an acute-care setting
- Pediatric and case management experience are helpful but not required - the team provides robust training
- Comfort with transparency, flexibility, and anticipating patient needs based on clinical pathways
- Strong critical thinking, analytical skills, time management, and ability to work autonomously
License / Certification - Active RN license in the District of Columbia (or eligible to obtain prior to start)
- Bachelor's degree required (Master's preferred)
- Certification in Case Management or Utilization Management preferred
