Job description

We are seeking a dedicated and compassionate Speech Therapist to join our Home Health team, providing vital therapeutic services directly in patients' homes. In this role, you will assess, diagnose, and treat communication and swallowing disorders across diverse patient populations, with a special focus on adult and senior population.This job is based in Nassau County. Your expertise will help improve quality of life for patients by delivering personalized therapy plans that promote functional communication skills and overall well-being.

This position offers an exciting opportunity to make a meaningful impact in a flexible, patient-centered environment. Responsibilities Conduct comprehensive speech and language assessments for patients of all ages, including children in early intervention programs. Develop and implement individualized treatment plans tailored to each patient's needs, goals, and medical history.

Provide direct therapy sessions focused on improving speech, language, voice, fluency, cognition, and swallowing functions. Collaborate with families, caregivers, and interdisciplinary teams to ensure consistent care and support at home. Monitor patient progress regularly and adjust therapy strategies accordingly to maximize outcomes.

Educate patients and their families on exercises, techniques, and strategies to promote ongoing improvement outside of sessions. Maintain detailed documentation of assessments, treatment plans, progress notes, and compliance with healthcare regulations. Qualifications Valid state licensure or certification as a Speech-Language Pathologist (SLP).

Prior experience working with senior population is highly desirable. Strong knowledge of speech therapy techniques, physiology, anatomy related to speech and swallowing functions. Experience conducting patient assessments using OASIS software and interpreting medical terminology.

Familiarity with early intervention programs Excellent communication skills to effectively collaborate with families and healthcare teams.
