Job description

Position Summary:
Under the leadership of the Director, Home Health, Oasis Review Nurse is an active member of the department that is responsible for prospective review of all oasis assessments ensuring all services and documentation meet the organizations standards and state and federal guidelines. Monitors for appropriate visit frequency and duration as per care guidelines.
Basic Qualifications:
  • Education:
Requires a degree in Nursing from an accredited school of nursing. A Bachelors' degree is preferred.
  • Experience:
Requires one to two years of work-related experience as a home health nurse, working knowledge for Oasis and ICD 9/10 coding. Knowledge of federal and state licensure requirements. Working knowledge of computer systems.
  • Licensure, Registrations & Certifications:
Requires a current license as a Registered Nurse and is listed in good standing with the state's Department of Professional Credentialing.
Requires and maintains a current certification in BLS.
Requires a valid driver's license and proof of insurance.
A Motor Vehicle Report is required upon hire.
Essential Job Responsibilities:
  • Review of Oasis assessments to include: Start of Care, Resumption of Care, Recertification, and discharge, to ensure appropriateness, completeness, and compliance with state and federal regulations.
  • Utilize SHP alerts and validation reports in review of oasis.
  • Ensure appropriate ICD coding and sequencing as it relates to the patient's medical record.
  • Consults with appropriate clinician to clarify any data integrity issues and or corrections to documentation.
  • Notifies agency leadership of problematic trends noted in oasis documentation.
  • Assists with other chart reviews and activities as requested.
  • Works with the clinical staff to ensure compliance with all state, federal, professional, HIPAA and JCAHO rules, regulations and guidelines.
  • Works with the staff to ensure to comply with all clinical care, safety, infection control and performance improvement measures as part of Home Health's efforts to provide quality care and services.
  • Responds to on-call requests as needed and participates in the scheduled rotation when needed.
  • Presents a professional image to CRHS's nursing co-workers and clinical and medical staff members, patients, families and visitors.
  • Attends professional develop programs and in-service training and meets the minimum requirement for continuing professional education.
  • Performs other related job duties as assigned.
