PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Lexington, Kentucky.

Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

  • Local Pay: $53.00 to $58.00/hour (W-2)
  • Travel Pay: $2,400 to $2,550 gross per week
  • Mileage Reimbursement: $0.49/mile (from first patient to last patient)

Location: Lexington, KY, 40503

Weekly Hour Guarantee: 32+
Productivity Expectations (points): 30
Coverage Areas: Bourbon, Clark, Estill, Fayette, Franklin, Harrison, Jessamine, Nicholas, Woodford (high need for coverage in Estill, Clark, Nicholas)
Shift schedule (days, start & end times): M-F, 8:00-4:30 PM
On Call/Weekend Requirement: yes, 1 weekend every ~4 weeks, 5-6 weekdays per month.

Responsibilities:

  • Deliver skilled home health nursing care by performing comprehensive patient assessments, medication reconciliations, Start of Care (SOC), ROC, Recertification, Follow-Up, and Discharge visits, developing individualized care plans, providing patient/caregiver education, and completing accurate OASIS and HCHB documentation within required timelines.
  • Manage a multi-county caseload across Bourbon, Clark, Estill, Fayette, Franklin, Harrison, Jessamine, Nicholas, and Woodford counties by efficiently scheduling visits, adapting to changing patient volumes, meeting productivity expectations, and providing coverage based on agency needs.
  • Collaborate with the interdisciplinary care team by communicating patient status changes, participating in weekly Wednesday case conferences and daily 8:30 AM Microsoft Teams morning huddles, coordinating care with physicians and therapists, and ensuring compliance with Medicare, agency, and regulatory standards.
  • Promote high-quality patient outcomes by using strong clinical judgment to identify patient risks, implementing evidence-based interventions, maintaining accurate documentation, supporting quality improvement initiatives, participating in the on-call rotation, and delivering compassionate, patient-centered care.

Must have qualifications/experience: HCHB experience is a must, 1-2 years of home health
What skills/attributes are nice to have, and will set a candidate apart? Strong independent clinical judgement & decision-making, excellent time management and route-planning, territory management experience (quickly learn geographical areas), strong OASIS knowledge with a history of accurate OASIS completion, thorough head-to-toe assessment with a complete medication reconciliation, problem solving mindset- finds solutions rather than escalating every challenge, reliable & accountable- meets productivity expectations, team-first attitude- supports agency goals rather than focusing solely on assigned visits.
Type of certification required: active & unrestricted RN license, valid driverโ€™s license & reliable transportation, current BLS certification
