Clinical Director (RN) – Medicare-Certified Home Health

Always Best Care | Kimura Home Health LLC Honolulu, Oahu, Hawaii | Full-Time | $120,000 – $140,000/year

About Us

Always Best Care is a locally owned Oahu agency providing Medicare-certified home health (skilled nursing, PT, OT, ST, HHA, MSW) and non-medical home care to kupuna across the island. We are a small, close-knit team that runs on the aloha spirit, modern tools (Microsoft 365, Kinnser/WellSky), and a commitment to doing right by our patients, families, and clinicians.

The Role

The Clinical Director (Director of Nursing) leads all clinical operations of our Medicare-certified home health agency. You will own quality, compliance, and clinical staff performance, and serve as the primary clinical decision-maker for the agency. Reports directly to the Administrator.

What You'll Do

Clinical Leadership

  • Oversee all patient care delivered by nurses, therapists, HHAs, and MSWs
  • Review and approve Plans of Care, OASIS assessments, and physician orders
  • Lead case conferences and ensure care coordination across disciplines
  • Serve as clinical escalation point for field staff, patients, and families

Compliance & Quality

  • Ensure ongoing compliance with Medicare Conditions of Participation (42 CFR 484), Hawaii DOH licensure, and accreditation standards
  • Lead survey readiness and represent the agency during state/CMS/accreditation surveys
  • Direct the QAPI program – track outcomes, Star Ratings, HHCAHPS, and drive performance improvement
  • Oversee OASIS accuracy, timely submission (iQIES), and coding/documentation integrity
  • Maintain policies, procedures, and clinical competencies

Staff Management

  • Recruit, onboard, train, supervise, and evaluate clinical staff
  • Conduct supervisory visits, competency checks, and annual evaluations
  • Manage clinician productivity and scheduling standards
  • Coordinate on-call coverage and participate in the administrative on-call rotation

Operations & Growth

  • Partner with intake and referral sources (hospitals, SNFs, physicians) to ensure timely starts of care
  • Manage clinical utilization, episode management, and LUPA avoidance under PDGM
  • Support payer requirements (Medicare, Medicare Advantage, HMSA, AlohaCare, VA, TriWest)
  • Help build and refine automated workflows – we value people who improve processes, not just follow them
What We're Looking For

Required

  • Active, unrestricted Hawaii RN license
  • Minimum 3 years of home health experience, including at least 1 year in a supervisory or management role
  • Working knowledge of Medicare CoPs, OASIS-E, PDGM, and home health QAPI
  • Strong documentation review, clinical judgment, and leadership skills
  • Valid Hawaii driver's license, reliable vehicle, and current auto insurance
  • CPR certification

Preferred

  • BSN or higher
  • Prior Clinical Director / DON / Clinical Manager experience in a Medicare-certified home health agency
  • Home health survey experience (state or accreditation)
  • OASIS certification (COS-C or HCS-O)
  • Experience with Kinnser/WellSky Home Health and Microsoft 365 (Teams, SharePoint)
  • Experience with Hawaii payers and local referral community
