Key Responsibilities

• Deliver individualized, family, and group counseling services to help patients identify and manage psychological and personal issues related to occupational and personal adjustment.
• Develop and implement individualized treatment plans based on psychiatric assessments, considering each patient's unique challenges and treatment requirements.
• Collect comprehensive bio-psycho-social information through interviews, case histories, and observational techniques to inform treatment strategies.
• Analyze patient data to establish diagnoses and treatment plans, and determine appropriateness for counseling or referrals to specialists when necessary.
• Evaluate the effectiveness of treatment plans, making necessary adjustments to optimize patient care and outcomes.
• Collaborate with interdisciplinary teams, including physicians, psychiatrists, and other healthcare professionals, to ensure a holistic approach to patient care.

Patient Population / Client Focus

The ideal candidate will work primarily with patients in hospice care, who may be experiencing complex emotional and psychological challenges associated with their health conditions and life transitions. You will be integral in helping patients and their families navigate grief, loss, and emotional distress, ensuring quality of life and emotional support during this critical time.

Qualifications

• Active and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• A Master's degree in Social Work, Counseling, or a related field is mandatory.
• Certification in Basic Life Support (BLS) is required to ensure safety standards are met.
• Demonstrated knowledge of medical coding, research methods, and documentation practices is essential.
• Proven ability to thrive in a fast-paced work environment, adapting to the needs of patients and collaborating effectively with a remote team.

Preferred Experience

A minimum of three years of clinical experience, particularly in hospice or palliative care settings, is preferred. Experience with diverse patient populations and knowledge of trauma-informed care practices will enhance your effectiveness in this role.

Compensation and Benefits
