Job Title:Program Manager- Registered Nurse

SUMMARY:

Develops and sustains an environment that supports excellence in clinical practice and patient care. Provides patients with psychosocial and continued care planning interventions. Serves as a community liaison and provides education in compliance with applicable hospital, departmental, and governmental regulations and policies. Plans and directs the operational, budgetary, and personnel activities of a designated area. Focuses on customer service as it relates to patients, visitors, family members, staff, and physicians, including working collaboratively with other disciplines to ensure an efficient, team-based approach to the delivery of patient care. Active member of the facility's Quality Improvement committee. Guides assigned areas in the utilization of recognized improvement tools, data analysis, and outcome measures.

RESPONSIBILITIES:

  • Receives referrals for patients at high risk for neurobehavioral continued care planning needs from the admissions team and in-house. Ensures that all inappropriate referrals are channeled correctly and documented.

  • Assesses patient needs and determines the mode of intervention. Interviews patients and/or families and records psychosocial assessments in accordance with policy, as documented in the chart

  • Implements the plan of intervention preparatory to discharge in compliance with departmental and governmental regulations.

  • Involves the patient/family in the treatment planning process as demonstrated through chart notes and signatures on the appropriate hospital forms.

  • Executes the plan of discharge/continued care, which is mutually agreeable to the patient/family.

  • Monitors and evaluates operations for compliance with licensure, accreditation, and regulatory standards.

  • Supports and facilitates staff professional development and participation in research programs.

  • Demonstrates leadership and influences others in both the internal and external environments to promote optimal care for individual patients and optimal health care services for the public.

REQUIRED QUALIFICATIONS:

  • Required Degree: Associate's degree in Nursing with minimal 4 years of RN experience

  • Required Certification/Licensure: Current NYS Registered Nurse license & Basic Life Support (BLS) certification or the ability to obtain one within 2 weeks of hire

  • Required Years of Experience: Minimum 1 years' experience working on a nursing unit.

PREFERRED QUALIFICATIONS:

  • Possess high level of communication skills

  • Excellent oral and written communication and interpersonal skills

  • RN BSN with minimal 3 years of RN experience.

  • Experience working on a high acuity unit that requires de-escalation techniques, behavioral intervention plans, and crisis response protocols.

PHYSICAL REQUIREMENTS:

Sedentary - Sedentary roles are primarily office-based and require prolonged sitting (67-100% of the workday) with minimal physical effort and lifting limited to under 10 lbs. Fine motor skills are necessary for computer work, writing, and telephone use, supported by clear visual and auditory ability. Cognitive and emotional demands are high due to extended concentration, frequent interruptions, independent decision-making, and information processing. Although physical exposure is low, administrative areas within healthcare environments may occasionally involve contact with infectious agents. Some roles may require flexible hours, TB screening, or a valid driver's license.
