We are seeking an experienced full-time Clinical Manager (RN) to lead our Stockton clinical team. This leadership role is responsible for overseeing the delivery of high-quality patient care, supporting and developing clinical staff, ensuring regulatory compliance, and partnering with interdisciplinary teams to achieve exceptional patient outcomes.

What You'll Do

  • Provide leadership, supervision, and support to a team of home health clinicians.
  • Coordinate the delivery of high-quality patient care while promoting positive patient outcomes.
  • Ensure compliance with all federal, state, local, Medicare, and Medi-Cal regulations.
  • Conduct and facilitate interdisciplinary care conferences, team meetings, and clinical in-service education.
  • Review referrals and coordinate timely patient assessments and admissions.
  • Make appropriate patient care assignments while overseeing clinician scheduling and on-call coverage.
  • Monitor clinical documentation and quality standards to ensure regulatory compliance.
  • Maintain accurate records related to patient assignments and clinical management.
  • Serve as a clinical resource, mentor, and preceptor for nursing staff when needed.
  • Foster a collaborative, supportive culture focused on excellence in patient care.

What You'll Bring

  • Current California Registered Nurse (RN) license in good standing.
  • Graduate of an accredited school of nursing.
  • Minimum of two (2) years of clinical nursing experience within the past five (5) years.
  • Previous home health leadership or supervisory experience preferred.
  • Strong knowledge of Medicare Conditions of Participation and home health regulations.
  • Excellent communication, organizational, and leadership skills.
  • Ability to build positive relationships with patients, families, referral partners, and team members.
  • Valid driver's license, reliable transportation, and current automobile insurance.
