Job description

Hourly Pay: $38.50 - $41.50 / per hour 

Registered Nurse (RN) – Transition Team 
Employment Type: Full-Time & Part-Time Available

About the Transition Team 

Our Transition Team is a supplemental care unit of highly skilled nurses whose primary focus is to quickly and efficiently support patients transitioning home while permanent nursing staff are actively recruited. This team is deployed immediately after a patient referral is received and the patient is admitted to Maxim. 

Transition Team nurses provide nursing care for all authorized hours in the home during a provisional period of 1–90 days. During this time, our office team recruits permanent caregivers. Once identified, those caregivers are given the opportunity to train and shadow alongside Transition Team nurses in the home before assuming independent care. After the transition is complete, Transition Team nurses move on to support the next new admission—providing variety, consistency, and meaningful impact in patient care. 

Responsibilities 

  • Utilize the nursing process to assess, plan, implement, and evaluate patient care 
  • Initiate and update plans of care as directed; request provider revisions as patient needs change 
  • Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals 
  • Respond to changes in patient condition and report to physicians or supervisors as appropriate 
  • Administer prescribed medications and treatments and document patient response 
  • Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning 

Requirements 

  • Current Registered Nurse (RN) license in the state of practice 
  • Current Health Certificate (per contract or state regulation) 
  • Current PPD or Chest X-Ray 
  • Current BLS certification 
  • Ability to travel 45 minutes to an hour  
  • Ability to work with pediatrics and adults  
  • Ability to care for (training available) patients with varied acuity (I.e. g-tube, trach, vent)  
  • One year of RN experience preferred
