Job description

Vivo HealthStaff has been retained to find Home Care Registered Nurses in San Jose, CA. This is a full-time position with an extensive benefits package.

DUTIES / RESPONSIBILITIES:
1. Clinical Practice: Evaluates, implements and develops a treatment plan for patients. Monitors patient changes and adjusts the treatment plans accordingly. Provides skilled nursing care as needed.
2. Instructs and supervises geriatric aides and LVNs in carrying out the prescribed home care treatment plan. The RN teaches and counsels patients and family members regarding family member's care and treatment.
3. Interdisciplinary Teamwork and Communication: Participates actively in the interdisciplinary team by offering clinical expertise in team planning and problem-solving. Communicates effectively with participants, family, and team members.
4. Other related duties as assigned.


QUALIFICATIONS (knowledge, skills, abilities)
1. Registered Nurse in the State of California
2. Two years of acute care experience required (ICU, ER or Critical care) or homecare/geriatric/nursing home experience
3. CA Driver's license and be WILLING to drive and travel.
4. CPR certificate
