RN Hospice - On Call

Job Description

The Hospice On Call RN is responsible for providing skilled nursing care, assessment, and support to hospice patients and families during non-business hours, including evenings, nights, and weekends. This role will ensure timely and compassionate response to patient needs, symptom management, and crisis intervention in alignment with the patient's plan of care.

Hours: Monday-Thursday 4:30pm-8:00am one week, Friday-Sunday 24-hour on-call rotation

Requirement: Active RN in the State of IL, Driver's License, Auto Insurance

This position is eligible for weekend differential and shift differential stackable on your base rate.

Responsibilities

Attend all Hospice patients at time of death., Collaborate with the interdisciplinary team (IDG) to adjust care plans based on patient needs and family concerns., Communicate with the on-call team, primary RN case managers, and medical directors as needed for follow-up or changes in care plans., Document all encounters accurately, thoroughly, and in a timely manner per agency and regulatory standards., Educate patients, caregivers, and families about disease progression, medication management, and comfort-focused care., Facilitate after hours admissions., Other duties as assigned., Participate in the hand-off process to ensure continuity of care between shifts and regular business hours., Perform comprehensive nursing assessments and implement interventions to manage pain and other distressing symptoms., Provide emotional support and guidance to patients and families in alignment with hospice philosophy., Respond promptly to on-call requests for patient visits, phone triage, and symptom management.
