Job Summary

Provide acute care, chronic care, preventive care and prenatal care to new and established patients of all ages in a primary care setting.

Job Duties and Responsibilities

  • Conduct detailed medical history assessment
  • Conduct physical exams
  • Medication reconciliation
  • Order diagnostic testing (e.g., lab work, EKG, cardiac echo, imaging procedures, etc.) when indicated
  • Coordinate specialty referrals as indicated
  • Provide consultation to patient and/or family to include necessity, prep, nature and effects of ordered diagnostic and therapeutic procedures.
  • Provide patient and/or family education regarding diabetes, nutrition, smoking cessation, pain management, etc.
  • Follow up on ordered laboratory studies, diagnostic procedures, etc.
  • Prescribe treatment or medication when indicated.
  • Document all activity in patient's electronic medical record to include comments about diagnosis and prognosis, diagnostic testing, patient condition, treatments and response to therapies and interventions, etc
  • Participate in clinic research activities if needed.
  • Perform telehealth visits if needed.
  • Work with a supervising physician, other providers and colleagues in the practice.
  • Perform any other related duties required to service primary care scheduled or walk-in patients

Job Eligibility Requirements

Education

NP: Graduate of an accredited MSN or DNP Nurse Practitioner Program

PA: Graduate of ARC accredited Master's Degree or Doctoral Degree Physician Assistant Program.

Certification

NP: NP-C (can be FNP, AGNP, AGPCNP, PNP-PC, etc.)

PA: PA-C

Both: AHA approved BLS certification is required before hire. ACLS and PALS must be completed after hire.

Licensure

NP: Current unrestricted RN license issued by or recognized by NCBON with approval to practice as NP

PA: Current unrestricted license issued by NCMB with approval to practice as a PA

Both: Federal and State DEA license with unrestricted prescriptive authority

Experience:

Experience working as a PA or NP in a primary care setting is preferred but not required

Job Duties and Responsibilities

  • Order testing (e.g., lab work, EKG, cardiac echo, imaging procedures, etc.)
  • Provide consults (diabetes, nutrition, smoking cessation, pain management, geriatric care, sleep apnea, anemia, and cardiology, etc.)
  • Prescribe treatment or services to further optimize the patient prior to planned procedure if needed.
  • Follow up on ordered laboratory studies, diagnostic procedures, etc.
  • Provide explanation as to the necessity and the results of ordered testing and procedures.
  • Participate in clinic research activities if needed.
  • Perform telehealth visits if needed.
  • Work with a supervising physician from the Department of Anesthesia.
  • Any other related duties required to service pre-surgical and/or pre-anesthesia care

Job Hours

  • FT, salaried position
  • Ability and willingness to work Monday - Friday 8a-5pm, providing up to 36 hours of direct patient care per week
