Job description

A Nurse Practitioner is sought for a PACE (Program of All-Inclusive Care for the Elderly) role in Anaheim, CA, serving frail seniors age 55 and older who meet nursing home level of care criteria but continue living at home under a capitated Medicare and Medi-Cal program.

The role. The NP functions under a collaborative practice agreement and the oversight of a Medical Director, providing primary care and professional medical services to PACE participants across multiple settings — including the PACE center, hospital, nursing facility, and participant homes, with frequent local travel required. Responsibilities include comprehensive intake assessments and reassessments at least quarterly, developing and monitoring individualized plans of care with the interdisciplinary team, evaluating and treating episodic illness, ordering and interpreting diagnostic tests, and performing procedures such as injections, immunizations, and EKGs. The NP prescribes within licensed authority, provides oversight of hospitalized participants in collaboration with a hospitalist — including discharge planning — makes home visits as needed, and participates in weekly intake and assessment meetings, daily morning meetings, family meetings, and clinical case conferences. The role also includes supervision of student interns and volunteers and participation in QAPI activities.

What we're looking for.

  • Graduate of an accredited Nurse Practitioner program
  • Current California RN and NP licensure through the California Board of Registered Nursing
  • DEA registration required within six (6) months of hire
  • Minimum five (5) years of nursing experience, including at least one (1) year with a frail or elderly population
  • Advanced physical examination skills
  • Current CPR/BLS and first aid certification
  • TB screening and completion of a health examination required prior to employment
  • Supervisory experience preferred
  • Interdisciplinary team experience preferred

Schedule. This is a full-time, day-shift position; the role spans multiple work settings and requires frequent local travel, with minimal out-of-town travel; use of a personal vehicle may be required.

Compensation. $140,000 - $160,000 per year.
