Are you a compassionate and detail-oriented Registered Nurse (RN) looking to make a meaningful impact in home healthcare? Excel Home Care Services is currently seeking a dedicated RN to join our team and provide Medication Management to clients in the comfort of their homes.

Position Overview

As a Medication Management RN, you’ll play a vital role in helping patients safely manage their medications. Through in-home visits, you'll assess needs, educate clients and caregivers, and collaborate with physicians and pharmacists to support the overall care plan.

Key Responsibilities

Conduct comprehensive in-home assessments to manage and review medications

Educate clients and caregivers on proper usage, side effects, and compliance

Pre-fill and organize medication planners as needed

Monitor for adverse reactions and report changes in condition

Coordinate with the care team to address any medication-related concerns

Accurately document visits, care provided, and patient progress

Perform health assessments including vital signs and ongoing care needs

Ensure compliance with healthcare standards and agency policies

Qualifications

Active RN license in the state (Required)

Experience in home health, medication management, or geriatric care (Preferred)
