Community Health-Referral RN
This is a multifaceted role that includes direct patient care, at multiple sites in partnership with NOVUS Health and Peter amp; Paul Community Services. The position will travel to multiple sites within the City of St. Louis providing basic physical reviews, medications monitoring, vital signs and assessing minor care issues. Requires the ability to document and chart patient information. This position requires flexibility in care delivery and the ability to perform multiple clinical care duties. In addition to the above role the individual will serve as a primary contact for NOVUS outside medical referrals and prior authorization services.
Report To: Clinical Nurse Supervisor
Duties amp; Responsibilities
***50% Travel within the city of St. Louis for this role. Mileage reimbursement paid out***
Community Patient Care:
• Assess new patients’ needs and chart incoming clients into different living sites coordinated through Peter amp; Paul Community Services.
• Be responsible for medication management and vital sign monitoring
• Screen, communicate, and respond to patient needs appropriately.
• Administer ordered medications via oral, subcutaneous, intramuscular, intradermal, or sublingual routes.
• Ensure patient privacy and comfort during the examination.
• Educate patients about health matters as needed.
• Ensure timely and accurate entry of medical information into EMR.
• Adhere to ethical nursing standards, policies, and procedures.
• Communicate to nurse practitioners and physicians on required medical information.
• Support the growth and advancement of Novus Health’s mission.
Referrals and Prior Authorizations:
• Assist NOVUS Health staff with medical and prescriptions that require prior authorization.
• Complete prior authorizations and close loop with referrals
• Coordinate ongoing contact with referral entities to ensure patient information is obtained for NOVUS medical record.
• Report to medical director, NP’s and Nurse Manager as needed and required
• Follow set timelines for case referrals and closures
Clinical Back-up Services:
• For Nurse clinical support at our Northside location
• Provide patient appointment medical assessment within EMR
• Other duties as assigned
Required Qualifications
• RN license in the state of practice
• Two (2) year’s experience in Public Health, clinical care and/or work in infectious disease.
• Nonprofit and health clinic experience preferred.
• Previous knowledge and training working with STI and HIV services
• Advanced communication skills; verbal and written.
• Excellent customer service skills
• Flexibility to work with diverse patient populations
• Ability to work within scheduled clinic hours of operation
