Job description

The Medicare/Skilled Nurse is a professional position responsible for monitoring, auditing and delivering skilled care services to those patients admitted to this facility or those who are a current resident that may be in need of skilled services. To ensure all documentation is appropriate, accurate and complete according to patient's condition.
QUALIFICATIONS:
Education: Graduate of an accredited School of Nursing or completion from an accredited institution for Vocational Nursing.
Relevant Experience:
Three (3) years of long term care experience preferred.
Computer Skills:
Intermediate computer skills
Licenses/Certifications: Current
LPN required, RN preferred from the State of TN

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  1. Physician's orders: Check/monitor admission orders for completeness/accuracy
  • Orders designate level of care
  • All medical diagnosis
  • Therapy orders
  • Equipment/devices and related orders(
    i.e. cpap & settings, Foleys & related orders, staple removal, etc.)
  • Date and time of admission accuracy
  • Labs needed, medical appointments
    made
  • All other as necessary
  1. Assist charge nurse in processing all admission orders
  2. Audit charts daily to ensure all physician's order are processed and carried out accurately
  3. Ensure all medications are available according to MD orders
  4. Complete/ensure completion of all nursing admission assessments
  5. Assess assigned patients daily with complete and accurate documentation
  6. Participate in the UR meetings weekly
  7. Participate in UR and care plan meetings weekly, ready to share pertinent information
  8. In-service staff as indicated and as it relates to skilled services
  9. Assist in coordinating follow-up appointments and D/C planning
  10. Complete 30 day discharge summary for skilled patients
  11. Review charts to ensure nursing and therapy documentation supports each other
  12. Nursing documentation should reflect tolerance and response to therapy each day
  13. Review nursing/therapy documentation on Mondays to ensure weekend documentation is present and appropriate.
  14. Follow up on reason therapy is not provided and take appropriate action
  15. Monitor lab orders and/or results notifying MD/family per patient needs and policy
  16. Monitor wounds, assess/obtain weekly measurements and complete weekly progress reports. This ensures your knowledge of all areas and their healing status.
  17. Notify MD/NP/family of changes in condition and/or physician's orders for skilled patients
  18. Cross train to assist with MDS/CP as needed/assigned
  19. Update care plans and CNA Communication Sheets daily according to MD order changes
  20. May be required to work as a charge nurse with med pass administration
  21. Will collaborate daily with MDS & therapy to ensure current knowledge of patient's status
  22. Will participate in the discharge planning process
  23. Supervise and delegate patient care needs to other nurses and CNAs
  24. Participate in Quality Improvement as assigned
  25. Perform other duties/tasks as assigned by the DON/ADON or Administrator

These duties/responsibilities are not all inclusive. There are others that may
be assigned to the Medicare nurse as determined by the DON/ADON/Administrator. The job duties may vary and will be based on the needs of the facility, the number of skilled patients and other relative information.
