Description
The Northumberland County Outpatient Clinic is seeking an independent contractor, to provide clinical outpatient therapy services to adults, children, and families.
The Outpatient Clinic is seeking self-motivated and caring licensed clinical outpatient therapist (must be a Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, or Licensed Psychological Associate) with experience providing therapy with adults, children, and their families in an office setting.
Duties:
  • Services may include conducting comprehensive clinical assessments, patient intake, obtaining psycho-social history, providing individual and family therapy, formulate treatment plans including strengths/needs assessment with measurable goals/objectives, develop crisis plans in conjunction with outpatient clinic participants, consult with other Human Services Agencies to obtain additional information regarding the participant, facilitate case conferences as needed with collateral supports and staff, provide crisis intervention services with individuals and their families experiencing acute stress related to behavioral health problems as needed based on the crisis plan devised with the participant, and provide after care coordination for participants including movement into and discharge from needed services.
  • The therapist will maintain the client schedule.
  • The therapist is responsible for obtaining appropriate authorizations.
The therapist must submit timely and complete notes, treatment plans, service orders, assessments, and other applicable documentations from all completed sessions as needed for reimbursement.
This position is a 1099 independent Contractor position. Length of contract is 1 year, at a rate of $40/hour.
