Community Liaison / Hospice Marketing Representative
Hospice of the North Coast Carlsbad, CA


Hospice of the North Coast is seeking a compassionate, driven, and relationship-focused Community Liaison to join our growing Partner Relations team. This role is ideal for a marketing professional who is passionate about hospice care, thrives in community outreach, and excels at building trusted partnerships throughout the healthcare continuum.
The Community Liaison serves as a key ambassador for Hospice of the North Coast, promoting our mission and services ethically and professionally within skilled nursing facilities, assisted living communities, physician offices, and the broader community.


What You'll Do
Develop and maintain strong referral relationships with physicians, skilled nursing facilities, Assisted livings, community partners and community organizations
Represent Hospice of the North Coast with professionalism, compassion, integrity, and a strong understanding of hospice philosophy and eligibility criteria
Execute strategic outreach plans focused on growing referrals and strengthening market presence
Conduct routine field visits, follow-up calls, in-services, educational presentations, and community events
Prioritize and manage high-value referral accounts through consistent relationship management and strategic planning
Utilize CRM systems (HubSpot) to document outreach activity accurately and consistently
Collaborate closely with admissions, clinical teams, and leadership to ensure smooth referral-to-admission transitions
Respond promptly and professionally to referral partner and community inquiries
Identify new business development opportunities while strengthening existing partnerships
Participate actively in community organizations, networking groups, and professional development opportunities
Monitor referral trends and maintain visibility within assigned territory

What We're Looking For
Minimum 2 years of healthcare marketing experience in hospice, home health, home care, palliative care, or senior services
Strong knowledge of hospice philosophy, Medicare guidelines, and referral processes
Demonstrated success building and maintaining referral relationships
Excellent communication, presentation, and interpersonal skills
Highly organized with the ability to manage multiple priorities independently
Experience using CRM platforms and tracking outreach metrics
Professional, self-motivated, and goal-oriented with strong follow-through
Ability to collaborate effectively with interdisciplinary teams
Bachelor's degree preferred
Valid driver's license, reliable transportation, and auto insurance required
