Job description

Title: Residential Support
Status: Non-Exempt/Hourly
Department: Residential
Supervisor: Program Manager
Supervision Exercised:  None

Work Schedule:  This is a non-exempt position and is subject to overtime.  Specific days and hours are to be scheduled by the supervisor to assure adequate coverage of responsibilities.  Schedules will be flexible and subject to change.  Schedules focus on clients’ needs and are expected to include evenings, weekends and overnight hours.

Training:  The person in this position is required to complete a minimum of 16 hours of pre-service orientation and training provided by Living Opportunities, Inc. upon employment and then to complete a minimum of 12 hours of pertinent in-service training annually


GENERAL STATEMENT OF DUTIES:

Provide support for people with disabilities that promotes and maintains independence, integration and coordination of services.  Independence includes skill acquisition, choice and control by persons served.


SPECIFIC DUTIES AND RESPONSIBILITIES:


Assessment, ISP and Support Services: 

Assist with required assessments and development of the individual support plan (ISP) for each person served at entry and at least annually thereafter.  Assist with the development of ISP support activities, based on the completed assessment(s) that promote and emphasize:

  • Choice and control by the individual
  • Self initiation and self monitoring systems
  • Maintenance and generalization of skills
  • Least intrusive and controlling approaches
  • Reduction of support needed over time

Specific supports may include assisting, monitoring, coordinating and training activities.  Support strategies are to be designed and implemented in ways that help the individual achieve increased physical and social integration in the community, especially making new friends and establishing new relationships.

Support design and implementation includes:

  • Analysis of the individual's needs and preferences
  • Design of support and training strategies that meet those needs and preferences
  • Adapting or modifying tasks so that individual can perform them successfully
  • Changing the physical or social environment to enable the person to be successful
  • Assessment of progress and redesign of support strategy based on assessment


Coordinate, monitor and perform supports necessary to implement the ISP.  Specific support areas may include, but are not limited to, helping individuals:

  • make new friends and establish new relationships
  • plan and prepare meals
  • monitor special diets
  • shop
  • wardrobe maintenance
  • administer medication
  • participate in all facets of leisure activities
  • budget and manage money
  • develop community safety skills
  • develop transportation resources and learn how to use available systems
  • make and get to appointments, including medical
  • clean their homes or teach cleaning skills
  • visit or correspond with friends and family.
  • with all facets of personal hygiene needs
  • arrange access to activities
  • care of personal property

Medical: 

Implement medical systems per policy and procedure, including evaluation and monitoring.  Provide liaison with individual's physicians and therapists as directed by supervisor.  Administer and review medications and treatments.  Store and dispose of medications per policy and procedure.  Encourage and support the individual's participation in all aspects of medical management, including self-management and self-administration training or adaptation.  Document medical activities per policy and procedure and supervisor's direction.

Safety:
Implement safety systems including fire drills and emergency evacuation practice.  Report or have repaired any emergency equipment that does not function properly or is in need of inspection.  Perform equipment and site maintenance and safety inspections as needed or directed.  Complete incident reports in timely manner, including required emergency notifications.  Report personal, co-worker or resident accidents, near-accidents and on-the-job injuries in a timely manner.  Encourage and support the individual's participation and self-management of skills and procedures that address personal safety concerns.  Document safety systems activities per policy and procedure and supervisor's direction.

Dietary and Housekeeping: 
Implement dietary and housekeeping systems including menu planning, shopping, food preparation and storage; cleanliness and maintenance of the living environment; and promotion of the individual's participation and self-management of skills and procedures that address housekeeping and dietary needs.  Document dietary and housekeeping activities per policy and procedure and supervisor's direction.

Financial and Property:
Implement financial and property oversight systems in a manner that both protects the individual's money and property, allows maximum access for the person and promotes the individual's participation and self management of money and property.  Document financial and property activities per policy and procedure and supervisor's direction.

Administration and Misc:

Maintain communication, correspondence and positive working relationship with individual, family members, case manager, physicians and other persons who provide support to individual.

Maintain records and documentation as required including data for training and support activities, house logs, medication and medical records, incident reports, menus, safety and cleaning checklists, emergency evacuation records, etc. per individual plans and agency procedure; and administrative documentation including time sheets, mileage sheets, etc.  Ensures that documentation is properly filed, organized and readily accessible for review at any time.

Maintain the confidentiality of persons served both in verbal communications and in sharing written material per agency policy and procedure.

Report in writing any accident, injury or significant unusual occurrence using the appropriate form (see policy and procedure for Incident Reports/Accident Reports).

Comply with agency policies and procedures and pertinent monitoring agency rules and regulations.

Attend meetings, participate in training and perform other duties as assigned and required by supervisor.
