About the Facility – Uintah and Ouray Health Center

Leave the hazards of urban life behind and apply your skills to a wide scope of health care. This ambulatory 40-hour facility is staffed by physicians, physician's assistants, nurse practitioners, and dental, optometry, and psychology providers. They provide comprehensive public health services including general medical, surgical follow-up, pediatric, prenatal and postpartum care. Dental, behavioral health, and optometry services are also available. Other clinical specialties are provided by visiting consultants and are scheduled periodically. Optometrist - Responsibilities
  • The optometrist will be responsible for full scope primary eye care delivered independently without assistance from text or nurses. This will include obtaining and documenting in the EHR in ophthalmic and medical history, a comprehensive ophthalmic exam, accurate and complete diagnosis, and effective treatment plans.
  • Treatment plans will include (as clinically indicated) a prescription of eye care, contact lenses, oral and ophthalmic medications, and ordering ancillary testing as needed.
  • Ancillary testing may include but not be limited to obsolic imaging studies (ultrasound, photos, OCT, pachymetry, Etcetera), visual field testing, laboratories studies, and radiographic imaging (playing films, CT, and MRI).
  • Minor surgical procedures such as foreign body removal and papilloma excision, and pre-operative and post-operative care of various ophthalmic surgeries, is also expected. In all cases documenting, performing, and arranging follow up and referrals as needed is required as well.
Optometrist - Requirements
  • Experience: A minimum of 2 years' experience as a clinical optometrist.
  • Education: Must have a Doctor of Optometry degree.
  • Licensure: Must have an active, unrestricted, and free of disciplinary action license as a nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
  • Certification: Must have a current certification in Basic Life Support.
