Position Summary
The Critical Care Outreach Nurse (CCON) is a resource for members of the multidisciplinary team and exists to improve patient safety and reduction of clinical risks through collaborative clinical assessment, consultation, and timely intervention to high risk patients. The CCON models expert clinical skills, promotes best practices, supports ongoing team education, and facilitates clear communication among team members, patients, and families. Collects data to assess performance and identify improvement opportunities in work flow for at risk or deteriorating patients.
As a Critical Care Outreach Nurse at Asante, you will have the opportunity to:
  • Work with medical/surgical floor charges nurses and health information systems to identify high risk patients and eliminate failure to rescue opportunities.
  • Assess patient needs and monitor patient response in collaboration with bedside RN.
  • Collaborate with health care team to promote best patient outcomes by providing education and guidance to team members.
  • Provide clinical consultation for complex critical patients in the ICU and CCU.
  • Assist and support staff with clinical decision making and troubleshooting patient care needs and issues.
  • Support critical care with implementation of best practice preventive measures to improve patient outcomes .
  • Partner with Emergency Department in time sensitive situations. Initially include target populations of septic patients, therapeutic hypothermia, fast track cardiac surgery and STEMI patients, and fast track to ICU patients (trauma, mechanical ventilation).
Qualifications
Education
  • Bachelor's degree in nursing preferred

Experience
  • 3 years critical care nursing experience required
  • Previous charge nurse experience preferred

Licensure/Certifications
  • RN: Registered Nurse licensed by Oregon State Board of Nursing (OSBN) required upon start
  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Association (AHA) required upon start
  • ACLS: Advanced Cardiac Life Support certified through the AHA required upon start
  • TNCC: Trauma Nurse Core Course within 6 months of hire
  • PALS: Pediatric Advanced Life Support within 6 months of hire

Preferred Certifications
  • CCRN: Critical Care Registered Nurse registered by American Association of Critical Care Nurses
