Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client's response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: Personal and professional accountability, Systems thinking, creative thinking, communicating and building relationships, Organizational and time management skills, Interpersonal skills, Intermediate level of computer skills.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications:

Completion of an accredited undergraduate nursing education program with at least 2 years of nursing experience. Recent experience in a community setting - 2 years Home Care experience preferred. BCLS-HCP from Heart and Stroke Foundation within the last 12 months. There is also an expectation to be a mobile worker. This role requires travel within the city of Calgary including caring for clients in their homes or in the community. You will be responsible to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work.

Preferred Qualifications:

Experience working with acute clients and/or long term clients. Experience in providing case management services. Canadian Nurses Association (CNA) certification in Gerontology. Experience in Home Parenteral Therapy (HPT), wound/skin and ostomy care, palliative support and client teaching.
