Physical Therapy (PT) - Home Health
The Home Health PT role delivers outpatient rehabilitation services in patients' homes, focusing on functional mobility, strength, and independence in daily activities. The PT plans care with the patient and family, adapting to the home environment.
The core purpose is to support safe, effective recovery and prevent unnecessary hospital readmissions through community-based therapy and education.
Responsibilities
- Assess home environment, functional status, and safety risks; develop an individualized plan of care.
- Deliver PT interventions, including therapeutic exercises, gait training, and balance activities at patient homes.
- Provide education on exercises, fall prevention, and activity modification for home settings.
- Monitor progress, document in the EHR, and communicate with the supervising PT and care team.
- Coordinate with caregivers for ongoing therapy and functional goals.
- Adapt plans based on home constraints, equipment availability, and patient progress.
Qualifications
Education:
Doctor of Physical Therapy (DPT) or, in some cases, Master's degree with licensure as PT; state-specific requirements apply.
Licensure:
Current PT license to practice in the United States.
Certifications:
- CPR/BLS certification required.
Experience or Skills:
Experience in home health or community-based PT; strong assessment, communication, and caregiver engagement.
Experience
- 2-4 years of PT experience; home health experience preferred.
- Experience coordinating with family caregivers and coordinating with other home health professionals.
Preferred
- Experience with telehealth or remote monitoring tools; knowledge of home modification and accessibility.
Work Environment
Primarily in patients' homes; travel required; documentation in mobile/remote EHR systems.
Flexible scheduling with potential for weekend and holiday coverage depending on patient needs.
