Job description

nit Details: PACE program serving a medically complex geriatric population in an outpatient and community based setting. Collaborative interdisciplinary care model focused on preventive care, chronic disease management, and improving the quality of life for elderly participants. Call 1-2 times a week.
Daily Patient Volume: 8- 12 Patients a day
Additional Providers/Support Staff: MA's & Support Staff
EMR System(s): Epic
Lic/Certs: Active CA Lic | BLS | DEA
BC/BE: BC
Credentialing Timeframe: 2-3 weeks after accepting assignment
Start Date: ASAC
Duration: 3 month commitment required, w/ option to extend preferred.
Shift: 5x8's M-F 8a-5p
Housing/Travel: All Inclusive
Pay Structure: Bid
Notes: 1hr for chart completion daily | *First week orientation will be 5x8's regardless of regular schedule and may include multiple sites*
Required Exp:
Will accept Physicians out of Residency. Any missing skills/procedures or limitations on seeing certain patients must be disclosed prior to presentation.
