Job description

Telephone Clinical Triage Registered Nurse

Your Health is seeking an experienced Registered Nurse to provide telephone-based clinical triage for patients in our growing primary care practice. The RN will assess patient symptoms remotely, determine the appropriate level and urgency of care using approved clinical protocols and nursing judgment, provide patient education, coordinate escalation to the appropriate provider or service, and document all calls and outcomes in the electronic medical record.

Then the essential functions should focus on:

  • Receive and assess inbound patient calls regarding symptoms and clinical concerns.
  • Apply approved evidence-based triage protocols and clinical judgment.
  • Determine the appropriate disposition, including home-care guidance, same-day provider evaluation, telehealth, urgent care, emergency care, or EMS activation.
  • Recognize red-flag symptoms and escalate urgent or emergent concerns promptly.
  • Coordinate with on-call providers, nurse practitioners, physicians, pharmacies, hospice, home health, and other care partners as appropriate.
  • Facilitate telehealth or telepharmacy visits when indicated.
  • Provide clear patient education and confirm understanding of instructions.
  • Document the assessment, protocol used, advice provided, disposition, escalation, and follow-up plan in the EMR.
  • Conduct follow-up outreach when required to support continuity and confirm resolution.
  • Participate in quality review, coaching, and root-cause analysis of adverse events.

QUALIFICATIONS

Qualifications

  • Current, unrestricted Registered Nurse (RN) license in good standing.
  • Minimum of three (3) years of clinical nursing experience preferred; triage, primary care, telehealth, or care coordination experience strongly preferred.
  • Demonstrated ability to assess clinical information, exercise sound judgment, and prioritize competing demands in a fast-paced environment.
  • Strong written and verbal communication skills with the ability to provide clear patient education and collaborate effectively with providers and care teams.
  • Proficient computer skills, including the ability to navigate multiple applications and systems simultaneously while maintaining accurate documentation.
  • Exceptional time management, organizational skills, and attention to detail.
  • Self-motivated with the ability to work independently in a remote environment with minimal supervision.
  • Ability to maintain patient confidentiality and comply with all HIPAA and organizational privacy requirements.
  • Dedicated home workspace that supports confidential patient communications and protects protected health information (PHI).
  • Reliable high-speed internet service and the ability to maintain consistent connectivity during scheduled work hours.
