Core Platform Engineer

Transform9

Birmingham, AL • On-site, Remote

Full-time

Medical, Dental, Vision, Retirement, PTO

Job description

Calling your doctor shouldn't be harder than seeing your doctor. But at most practices, patients wait on hold, front-office staff drown in ring queues, and revenue walks out the door with every missed call.

Transform9 builds AI agents that answer every call for physicin practices - scheduling appointments, routing callers, and resolving routine requests in seconds, around the clock. Our agents integrate directly with each practice's EHR, practice management, and phone systems, and adapt to how their front office actually works.

Our agents have handled millions of patient calls for thousands of providers, cut average hold times by 90%, and decreased call abandonment by 80%. We're headquartered in Birmingham, AL, and growing fast.

About the role

The Core Platform team is the heartbeat of Transform9 - it owns every inbound and outbound call flow, all real-time voice processing, and every end-user touch point a patient or provider encounters. As a senior engineer and technical lead on this team, you'll set direction, drive architectural decisions, and own the reliability of a platform that practices depend on every minute their phones ring.

What you'll do
  • Lead technical design and architecture for the Core Platform - the real-time voice pipeline, conversation orchestration, scheduling logic, and call routing engine
  • Own features end-to-end: design, implementation, tests, deployment, and monitoring
  • Set and uphold engineering standards through code review, design feedback, and documentation
  • Partner closely with product and infrastructure to keep the platform scalable and highly reliable
  • Serve as the primary escalation point for production issues affecting inbound and outbound call flows
  • Mentor and grow engineers on the team; help shape the team's process and culture
  • Participate in on-call rotation and contribute to incident retrospectives
  • Develop production-grade software that meets HIPAA and healthcare security standards
  • Collaborate with Customer Success and Product to understand operator workflows and translate them into great UX
Location

Remote (US). If you're in the Birmingham, AL area, this role is hybrid from our office (3 days in office / 2 remote).

Requirements

  • 10+ years building production backend or distributed systems
  • Demonstrated experience leading or tech-leading an engineering team (formal or informal)
  • Deep proficiency in a modern backend stack (Python, TypeScript, or equivalent)
  • Strong background in real-time systems, event-driven architectures, or telephony/voice platforms
  • Track record of owning complex systems end-to-end and driving them to production
  • Strong communication skills - able to align stakeholders, write clear specs, and mentor peers
  • Comfort operating in a fast-moving, ambiguous environment
Nice to have
  • Experience with voice/telephony platforms (SIP, WebRTC, media servers)
  • LLM-based product or conversational AI experience
  • HIPAA-regulated or other compliance-heavy environment experience
  • Experience in healthcare technology or patient-facing systems

Benefits

  • Top-tier health coverage: medical, dental & vision
  • 401(k)
  • Paid time off + holidays, and paid parental leave

If you want to fix the front door of healthcare, we'd love to hear from you.
