CHRONIC CARE REGISTERED NURSE POSITION SUMMARY:

The chronic care nurse monitors patients from intake to the facility and ensures continuity of care leading up to and following release into the community by advocating for patient needs while acting as a liaison between the multiple systems impacting the lives of offenders to provide a proactive and collaborative approach in serving the patient population in a correctional environment.

CHRONIC CARE REGISTERED NURSE MINIMUM REQUIREMENTS:

  • Graduate of an accredited School of Nursing
  • Active licensure as a Registered Nurse in the state of employment. (Delaware or multistate)
  • Possesses an active CPR certification.
  • Remains knowledgeable about specific state laws and regulations governing practice.
  • Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.
    CHRONIC CARE REGISTERED NURSE ESSENTIAL FUNCTIONS:
  • Maintain accurate, detailed reports and records..
  • Administer medications to patients and monitor patients for reactions and side effects.
  • Monitor, record, and report symptoms or changes in patients’ condition.
  • They coordinate with outside healthcare providers, schedule appointments, and ensure that necessary medical records are prepared.
  • Alongside clinical duties, RN Telehealth Coordinators may have administrative tasks such as scheduling appointments, maintaining records, and coordinating with external healthcare providers.
  • Additional duties as assigned by facility
