Position Summary

The Licensed Practical Nurse (LPN) provides nursing care and supervision to residents, coordinates with the interdisciplinary team, and ensures high-quality care in accordance with physician orders and facility policies.

Responsibilities

  • Provide direct nursing care including treatments, wound care, and medication administration in accordance with orders and scope of practice
  • Supervise and support CNAs/CMAs, delegate tasks appropriately, and ensure completion of care
  • Monitor resident conditions, complete focused assessments, and promptly report significant changes to the RN and/or provider
  • Participate in care planning, documentation, and resident/family education as needed
  • Ensure accurate and timely documentation in the medical record and required logs
  • Promote infection control, safety, and regulatory compliance on the unit
  • Foster a positive, team-oriented environment focused on resident outcomes and satisfaction

Qualifications

  • High school diploma or equivalent
  • Current, unencumbered LPN license in the state of Tennessee
  • Long-term care/skilled nursing experience preferred
  • Strong assessment, critical thinking, and communication skills
  • Ability to lead and mentor team members in a fast-paced environment
