About the Opportunity
We are seeking a Skilled Nursing Physical Therapist (PT) to join a quality-driven SNF focused on restoring mobility, independence, and quality of life for short-term rehab and long-term care patients.
This role is ideal for clinicians who value 1:1 patient care, interdisciplinary collaboration, and meaningful functional outcomes. New Grad PTs welcome with mentorship available.
What You'll Do
  • Evaluate and treat patients recovering from illness, injury, or surgery
  • Develop and update individualized plans of care (POC)
  • Provide 1:1 skilled therapy to primarily Medicare patients
  • Collaborate with nursing, OT, SLP, and rehab leadership
  • Document evaluations, progress notes, and discharge summaries
  • Educate patients and families on mobility, safety, and recovery
Qualifications
  • Doctorate or Master's Degree in Physical Therapy
  • Active (or pending) state PT license
  • SNF experience preferred, not required
  • Strong clinical reasoning and communication skills
