This full-time, remote position offers the opportunity to work within a Hospice Center in Pasadena, California. Key Responsibilities
  • Conduct thorough evaluations and assessments to determine the social, emotional, and environmental needs of clients.
  • Monitor progress and outcomes through regular follow-up assessments and collaboration with team members.
  • Offer essential support to families, equipping them with resources to enhance their caregiving abilities.
  • Build trusting relationships with children and families, creating a nurturing atmosphere conducive to personal development.
  • Implement evidence-based practices, including the use of the coaching model and natural environment teaching.
  • Ensure compliance with all safety standards and practices to protect employees and clients in the hospice setting.

Qualifications
