• Conduct thorough evaluations of patients' physical abilities, setting measurable goals and developing personalized treatment programs in accordance with physician recommendations.
• Monitor patient progress and adapt treatment plans based on individual responsiveness to therapy, ensuring interventions remain effective and are tailored to evolving needs.
• Maintain accurate and up-to-date records documenting evaluations, treatment outcomes, plans of care, and discharge information, demonstrating the necessity of therapeutic interventions using appropriate medical terminology.
• Supervise and mentor Physical Therapy Assistants (PTAs) and support personnel, providing guidance in their delivery of patient care to ensure optimal patient progress.
• Collaborate with a diverse team of healthcare professionals, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate and assess comprehensive intervention programs.
• Engage in ongoing education about best practices in therapy techniques and patient management, contributing to department best practices and productivity standards.

As a Physical Therapist at Heartfelt Life Insights Center, you will primarily work with patients recovering from surgical procedures, injuries, neurological conditions, and chronic pain disorders. Your role will be vital in assessing their impairments and functional limitations, applying evidence-based practices to facilitate recovery and functional independence.

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Active Physical Therapist license issued by the State of Virginia.
• Strong written and verbal communication skills, essential for effective patient and team interactions.
• Proficiency in word processing and electronic health record systems to maintain patient documentation.

• A minimum of 2 years' experience in a physical therapy setting, complemented by practical exposure to outpatient rehabilitation practices.
• A background in managing diverse patient populations, particularly those requiring post-operative rehabilitation or chronic disease management.
