Job Description
The Pharmacist plays a vital role in advancing the community's health and wellness by ensuring safe, effective, and affordable access to medications. This position, working as part of a multidisciplinary care team within a Federally Qualified Health Center (FQHC), combines clinical expertise with a deep commitment to building trust, improving health literacy, and addressing social determinants of health.
KEY ACCOUNTABILITIES:

  6. Supports the organization as needed.
KNOWLEDGE, SKILLS, AND ABILITIES:
  1. Meticulous accuracy is essential to prevent medication errors.
  2. The ability to clearly and effectively communicate with patients and other healthcare professionals in a diverse healthcare environment.
  3. The capacity to evaluate medication-related issues and make sound clinical decisions.
  4. A genuine concern for patient well-being and the ability to work with people who are ill or in distress.
  5. The ability to manage a busy workflow, supervise staff, and maintain a well-organized pharmacy.
Qualifications
QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:
  1. Doctor of Pharmacy (PharmD) degree or B.S. from an accredited college of pharmacy.
  2. Active and in good standing Arizona pharmacist license.
  3. Two or more years of experience in pharmacy, preferably in a community health center or public health setting, with knowledge of the 340B program operations and compliance.
  4. Must possess a valid and unrestricted Arizona driver's license (within 30 days of hire if the applicant is from out of state) with no DUI/DWI or reckless driving convictions in the past five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver's license during employment. Must be insurable under NATIVE HEALTH's liability auto policy.
  5. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the first ninety (90) calendar days of employment and maintain a valid card for the duration of employment with NATIVE HEALTH.
  6. Must pass substance abuse testing upon employment and submit to random testing during employment.
  7. Must provide an up-to-date immunization (IZ) record upon employment.
  8. Must have a current Tuberculosis (TB) skin test report within the past 12 months upon employment and provide an annual update.
  9. Must obtain Basic Life Support (BLS) certification within 90 calendar days of employment and maintain certification during employment.
