Summary Statement
This Casual/Seasonal Adolescent Treatment Services position sits within the Child and Family Care Coordination Unit and will be a member of the unit's team. Care Coordinators support children and families with care coordination, and link families to resources and services that are clinically necessary in a least restrictive environment to promote positive emotional and behavioral adjustment. This position works closely with the family's formal (e.g., child welfare, juvenile justice, schools, etc.) and informal supports (e.g., neighbors, family members, clergy, etc.) to improve the academic, social, emotional, and behavioral outcomes for their child(ren).\r\n
Essential Functions
Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class.Since class specifications are descriptive and not restrictive, incumbents can complete job duties of similar kind not specifically listed here.\r\n\r\n Participates in treatment planning and discharge planning in conjunction with the facility personnel as well as with members of other involved agencies to insure that the treatment results in the expeditious release of the adolescent patient.\r\n Designs the aftercare treatment plans and referring clients to appropriate outpatient treatment facilities, community support systems, and/or residential treatment facilities and insures that the discharge recommendations and aftercare plans are carried out.\r\n Interacts with clients' families interviewing members, observing family dynamics, assessing strengths or dysfunctions and adapts aftercare treatment plan to reflect their needs as well as those of the client.\r\n Provides direct supportive services including home visits, individual and family counseling as well as family crisis intervention.\r\n Monitors the outpatient treatment components to insure progress and resolve any problems that might arise.\r\n Develops and maintains a 'bank' of community resources to provide the necessary services for the adolescent and family.\r\n Prepares monthly statistical, narrative and court reports, as required detailing caseload activities.\r\n Prepares progress notes on each client.Participates in staff meetings.\r\n Makes formal presentations before community groups, schools and/or professional organizations.\r\n\r\n
Job Requirements
JOB REQUIREMENTS for C/S Adolescent Treatment Services Coordinator\r\nApplicants must have education, training and/or experience demonstrating competence in each of the following areas:\r\n\r\n Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.\r\n Six months experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individuals human service needs.\r\n Six months experience in making recommendations as part of a clients service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.\r\n Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.\r\n Six months experience in narrative report writing.\r\n Possession of a valid Driver's License (not suspended, revoked or cancelled, or disqualified from driving).\r\n\r\n
