Job Description:
1.Provides safe, efficient and high quality personal care services to assigned patients under the supervision and guidance of a registered nurse, and in accordance with Hospice policies.
2.Performs duties in accordance with the patient's plan of care and appropriate to the level of training, including but not limited to, the following:
a. monitoring vital signs, as assigned, including: blood pressure, temperature, pulse and respirations;
b. assisting patients with bathing and personal hygiene needs;
c. assisting patients with dressing and undressing;
d. assisting patients with mouth, hair, nail and skin care;
e. prepares and assists with meal preparation, as assigned, in feeding of the patient, and in clean up after the meal;
f. assisting patients with transfers or ambulation;
g. assisting patients with range of motion exercises;
h. making beds, occupied and unoccupied;
i. administering enemas and suppositories, as directed;
j. assisting with ostomy or catheter care, as directed;
k. assisting patients with toileting needs;
l. reporting accurately and promptly to the registered nurse, any changes in condition, level of function, or mental status;
m. able to work autonomously.
3.Attends all mandatory meetings and in-service education programs.
4.Provides accurate and timely documentation of all services rendered and keeps accurate time sheets.
