Job Description:
Are you passionate about making a positive impact on patients outside the hospital setting? If so, the Peshtigo My Home My Care RN role may be the perfect fit for you!
In this position, you will collaborate with our dedicated Peshtigo My Home My Care Team to coordinate care for complex patients receiving primary care services in their homes, Assisted Living, or Skilled Nursing Facilities. As the primary liaison between complex patients and various healthcare entities, you will work within your scope to enhance patient outcomes, compliance, and satisfaction. You will partner closely with our two department providers to identify patients who require enhanced healthcare navigation and education, ensuring seamless coordination of care across the continuum. This role also provides vital support to patients and their families by offering education and resources to improve their overall health and wellbeing.
As an RN, you will develop and implement patient-specific goals aimed at reducing care gaps and barriers for your managed patient panel. Additionally, you will receive training in our triage office to assist with office support and triage care needs for the same patient population. There may also be opportunities to provide limited lab services for homebound patients receiving our care.
The care coordinator will operate within the full scope of nursing practice, adhering to the Wisconsin Nurse Practice Act, Administrative Code, and relevant nursing standards, as well as Emplify Health Policies and Procedures.
The Registered Nurse (RN) is responsible and accountable for providing quality professional nursing care to patients and their families and significant others adhering to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing, and/or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses and Bellin Health Policies and Procedures. The responsibility includes providing a safe environment for patients, families, staff, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for the patient. Community involvement and collaboration is encouraged.
Qualifications:
  • Current Wisconsin RN license required.
  • Michigan RN license must be obtained upon hire if not already held.
