Position Summary
The Hospice RN Clinical Coordinator is responsible for carrying out administrative and management functions and oversight in the absence of the Director of Clinical Services. May carry caseload due to ebb and flow of patient census. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.
Position Specific Responsibilities
  • Promotes a positive working relationship between team members and program leadership, agency and corporate personnel.
  • Promotes positive working relationship with family, physician and other facilities.
  • Models desired behaviors including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment.
  • Attends and participates in staff meetings, educational programs and community events, as requested.
  • Promotes Hospice philosophy.
  • Communicates identified needs and potential solutions to Area Executive of Clinical Operations ACO and Hospice Director of Clinical Services.
  • Assists with onboarding, provide mentorship to new staff.
  • Assists with DME and medical supply orders to ensure quality and advise Director of Clinical Services of any trends and receiving approval for out of formulary items from ACO or DCS.
  • Monitors supply closet for expiration dates.
  • Manages medication orders to ensure needed meds are available for any admissions. Fax to providers as needed. Receive approval for any out of formulary items with ACO or DCS.
  • Submits requests for medical records and hospital inpatient charts, and processes provider notifications when relevant.
  • Answers incoming telephone calls / clinical calls, takes accurate messages, and transfers calls appropriately.
  • Assists DCS with Interdisciplinary Team meetings, including agendas, update coordination notes as needed.
  • Provides coverage for DCS as needed.
  • Processes HCHB workflow timely for admissions, recertifications as directed by ACO or DCS, and new Physician orders.
  • Supports team scheduling to meet patient care plans including HOPE, GAP, and HVLDL visits
  • MUSE Champion to oversee changes in patient's acuity and required daily visits
  • Actively participates in Quality Assessment and Performance Improvement (QAPI) activities.
  • Participates in the development and update of patient care policies and emergency procedures.
  • Make post-admissions calls if directed by ACO
  • Documents in the patient's clinical record when required; demonstrates proficiency with documentation requirements.
  • Participates in care delivery for patients as needed including conducting admissions, recertification visits, routine visits, and discharge visits.
  • Manages the care for a caseload of patients if needed to support ebb and flow of census changes.
  • Participates in after-hours shifts as needed including primary and back-up on-call, performs patient admissions and visits as needed.
  • Makes visits to the hospital or customers as needed for additional educational support.
  • Performs RN Case Manager responsibilities as needed and may carry a caseload due to patient census ebb and flow.
  • Performs other duties as assigned
Education and/or Experience
  • Active and unencumbered Registered Nurse License required.
  • One (1) year of Hospice experience required.
Skills
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
Certifications, Licenses, and Registrations
  • Valid driver's license and auto liability insurance.
  • Current Registered Nurse State License as required.
  • Current CPR certification.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions.
