Job description

Job Title:ย Social Worker (MSW)ย 
Facility:ย Holy Cross Hospital Silver Spring
Location:ย ย 1500 Forest Glen Rd., Silver Spring, MDย 20910
Duration:ย 13ย weeks
Shift:ย 8:00 am - 4:30ย pm (M-F)ย 
Pay Rate:ย $50/hr (W2) | $53/hr (1099)ย 

Position Overview:
This role is essential in supporting patients and families through the discharge planning process and facilitating smooth transitions of care.
The ideal candidate will have at least 3 years of recent hospital discharge planning experience and be comfortable working with adult, pediatric, and maternal health populations, including individuals with complex psychosocial needs such as homelessness or behavioral health concerns.

Key Responsibilities:
  • Conduct admission assessments to identify patientsโ€™ psychosocial needs
  • Develop, document, and implement individualized discharge plans
  • Coordinate post-discharge services including:
    • Home health care
    • Subacute and acute rehab placements
    • Community resources for homeless or at-risk patients
  • Work collaboratively with care managers, nurses, physicians, and interdisciplinary teams
  • Attend daily discharge planning meetings
  • Actively communicate with patients and families to provide education, support, and planning
  • Ensure compliance with hospital, state, and federal discharge regulations

Unit/Work Environment:
  • May work in a dedicated inpatient unit or rotate across multiple units
  • High interaction with patients, families, and interdisciplinary healthcare teams
  • This is not a therapy or counseling role โ€” focus is on care coordination and discharge readiness

Minimum Requirements:
  • Masterโ€™s degree in Social Work (MSW) from an accredited institution
  • 3 years of recent hospital-based discharge planning experience
  • Experience working with:
    • Adult, pediatric, and maternal health patients
    • Homeless populations and complex psychosocial cases
  • Strong communication and interpersonal skills
  • Ability to work independently in a fast-paced hospital environment
